  1. Prevalence ratio (PR) for 100% positive assessments and for 0–49% positive assessments associated with different patient characteristics. Crude PRs are adjusted for patient clustering. Patient-adjusted PRs are adjusted for patients' gender, age, frequency of attending a general practitioner (GP) and self-rated health and patient clustering. Fully adjusted PRs are adjusted for GPs' gender, age, weekly working hours, organisation of practice and staff in addition to patients' gender, age, frequency of attending a GP, self-rated health and patient clustering. The goodness of model fitting is shown regarding the fully adjusted PRs (p-values).
  2. * Patients who marked 100% of the answered questions in one of the two most positive answering categories.
  3. Patients who marked less than 50% (0–49%) of the answered questions in one of the two most positive answering categories.
  4. Prevalence
  5. § Crude prevalence ratio is the unadjusted prevalence ratio adjusted for patient clustering
  6. || Patient-adjusted prevalence ratio is the prevalence ratio adjusted for patients' gender, age, frequency of attending a GP and self-rated health and patient clustering.
  7. Fully adjusted prevalence ratio is the prevalence ratio adjusted for GPs' gender, age, weekly working hours, organisation of practice and staff in addition to patients' gender, age, frequency of attending a GP, self-rated health and patient clustering.
  8. ** 95% confidence interval for the fully adjusted prevalence ratio.
